Plot Outline: Following Jigsaw's grisly demise, Mark Hoffman, the final apprentice to the serial killer is deigned a hero. Meanwhile, Agent Strahm is tested and puts the pieces together. While Strahm realizes that Hoffman is helping Jigsaw, five seemingly unconnected people face a horrible lesson about teamwork.

Plot: Seth Baxter (Joris Jarsky), a convicted murderer who was released early from prison on a technicality, is chained to a table beneath a pendulum blade. A videotaped message informs him that in order to survive, he must crush his hands by putting them into two presses and pushing the buttons inside. Even though he does so, the pendulum swings down and slices him in half. The fact that the trap was inescapable marks it as a creation of someone other than Jigsaw (Tobin Bell).
In a scene from the end of Saw IV, Agent Peter Strahm (Scott Patterson) enters the room where Jigsaw died and shoots Jeff Reinhart (Angus Macfadyen) dead in self-defense. He then looks around and discovers the bodies of Jigsaw and Amanda Young. It is at this time that he hears the door slam shut, and he is locked in by Lieutenant Mark Hoffman (Costas Mandylor). Strahm soon finds a hidden door that leads into a passage. Here a microcassette recording urges him not to proceed any further; ignoring it, he moves down the passage and is attacked by a figure in a pig mask.
Goofs:
- Errors made by characters (possibly deliberate errors by the filmmakers): In the film, Erickson is constantly wearing a BlueTooth headset. When Hoffman calls him from Strahm's mobile phone, he answers the phone as normal, not using the headset. He appears to repeat this when he tries to track Strahm's mobile phone.
- Boom mic visible: In the scene when Jill (Betsy Russell) is talking to the FBI Agent in the Office, a huge boom shadow can be clearly seen above their heads
- Incorrectly regarded as goofs: In the bathtub trap room, the three surviving characters are told that they have three minutes to "bridge the gap" and unlock the door to the next room. A timer above the door shows 15 minutes, but this one is connected to bombs in the corners of the room (something not mentioned on the videotape); the three-minute timer is set up separately.
- Incorrectly regarded as goofs: The tank around Agent Strahm's head did not necessarily need to be airtight. It was secured in an upright position by two metal bars attached to the ceiling, and could therefore have had an air hole in the top panel that would not have prevented death by drowning.
- Crew or equipment visible: (At 00:09:40) On the unrated directors cut during the cube trap, while the camera is panning around you can clearly see the top of the set and see the ceiling of the studio they are filming in.
- Factual errors: About 7 minutes into the movie, when Peter Sthram is walking through the building with his gun raised, and he is checking all of the corners in the building, he holds the flashlight on the wrong side. Police are trained to hold the flashlight on the opposite side of the gun, with the left hand going under the right hand. So, for example, if you are right-handed, the flashlight, held in the left hand, goes on the right side.
- Revealing mistakes: In the part where Agent Strahm is in the room where Jigsaw was killed, when he (Strahm) unveils his phone, he tries to phone someone. When it says 'NO SIGNAL FOUND' if you look carefully at the top-right hand corner, it has 3 bars of reception.
- Continuity: In the room with the mounted razor traps, a timer on the wall is shown to begin counting down from 15:00. However, a few seconds later Charles points to the timer, and it is shown with 15:00 on its display again. The beeping heard once the timer has begun also stops in the next few shots.
Trivia:
- The scene where Hoffman leaves the Gideon plant with Corbett and says "nobody made it" was filmed for, and edited out of, Saw IV (2007). Behind-the-scenes footage of this scene appears in Darren's Video Diary on the DVD and Blu-ray of Saw IV.
- The first sequel in the Saw franchise to not open in first place on opening weekend at the US box office.
- Danny Glover was offered a chance to reprise his role in a flashback. He had to decline because of shooting conflicts with the movie Blindness (2008).
- Seth's trap is inspired by the Edgar Allen Poe short story "The Pit and the Pendulum."
- When Agent Erickson talks to Jill Tuck, the picture on his desk is a real photo of actor Mark Rolston and his wife.
- The role of Dan Erickson was written for 'William Forsythe'. He declined the offer due to scheduling conflicts.
- Producer Oren Koules owns the Tampa Bay Lightning hockey team. Goalies Olie Kolzig and Mike Smith wore Saw-themed masks for two weeks when the movie opened. The masks were then auctioned off.
- The first film of the series in which Billy the Puppet does not appear in person (excluding flashbacks).
- Real animal blood was used for the final trap. The director said they wouldn't have used it if they'd known how bad it was going to smell.
